1. To apply for a new license, go to http://www.flhealthsource.com/
2. Click on ‘Apply for Initial Licensure’ button on the home page
3. If you have registered or applied at the website before, select your board/council, profession, and enter your email address and password. Then click on ‘Log In’
4. If you are new at Florida Health MQA Services website, click on ‘Create Account’
5. Select your Board/Council and your Profession
6. Enter your Email address and preferred password
7. Click on ‘Create Account’ to complete the registration process.
8. Now log in to the website and click the ‘apply for licensure ‘ link
9. Provide the required details carefully and follow the instructions on screen to complete your application process
