You do not have to go to FDA offices to get the correct for forms from the FDA, because all you need is a working laptop or personal computer with a reliable Internet connection.

Then, follow these simple step-by-step instructions:

  1. Visit www.fda.gov and click on “Reports, Manuals & Forms,” which is found under “About FDA.”
  2. On the left, there are several reports, manuals and forms boxes. Click on the forms tab.
  3. You will be prompted with a list of downloadable FDA forms.
  4. Make sure that you have Adobe Reader installed on your computer, then, under the form column, choose the form that you want to download by clicking on the highlighted code.
  5. The form you choose will display a preview version. Then,  download the form and save it to your computer’s hard drive.
